Support at Home with BASSCARE

Support at Home is the new national approach to in-home support, bringing earlier programs together into one clearer, more flexible system.

The Support at Home Program provides eight support levels, from light assistance to more comprehensive support, so services can adapt and grow as life changes.

Getting Started

You do not need to wait until everything is in place. If you are just beginning to think about support at home, you can contact BASSCARE at any stage. Our team can help you navigate the assessment pathway, explain what to expect, and guide you through accessing the Support at Home Program through My Aged Care.

While waiting for your Support at Home package, support does not need to be on hold. Our services can be funded through Support at Home, the Commonwealth Home Support Programme (CHSP), or paid for privately, giving you flexibility to start receiving support at home when you need it.

Once an assessment has taken place and a Support at Home Package is assigned to you, choose BASSCARE as your aged care Support at Home provider. We will work with you to set up services that fit comfortably into your daily life, at a pace that feels right.

Your Pathway To Home Services

How Our In-Home Care Services Work

Assessment

Register with My Aged Care and receive an assessment (ACAT) to determine your care needs and eligibility.

Approval & Waiting Period

Once approved, you will receive a notice of decision confirming your eligibility and indicating your priority level. You may need to wait until a package becomes available based on your priority and availability.

Finding a Provider

When a package is assigned to you, you have 56 days to choose a home care provider. BASSCARE Home Care is here to support you with a range of services tailored to your needs.

Create a Care Plan

Together with BASSCARE, you will develop a personalised care plan outlining the services and support you require.

Service Delivery

Once your care plan is in place, BASSCARE will coordinate and deliver the agreed services, regularly reviewing and adjusting your care as needed.

Our trusted staff deliver a wide range of Support at Home services, offering practical help with a steady, familiar presence. Our approach to providing support at home ensures that assistance is practical, respectful and responsive to your changing needs. 

When needs extend beyond what we provide directly, we partner with an extensive network of providers, ensuring you have access to the full range of support available through the system. 

As a local provider of Support at Home, we focus exclusively on Boroondara and surrounding suburbs. This means you are supported by a team who understands the local community and is close by when you need them.

Support at Home groups funded ongoing aged care services into three main categories, each with defined service types.

Each category is tied to distinct funding arrangements and co-contribution rates, with clinical supports generally fully funded and independence/everyday living services subject to client co-payments according to assessed ability.

  • Nursing care (delivered by registered nurses) 
  • Allied health (physiotherapy, occupational therapy, podiatry, etc.) 
  • Clinical advice and case management 
  • Restorative care (for regaining or maintaining functionality) 
  • Palliative/end-of-life care 


Independence 


  • Personal care (help with daily living activities) 
  • Social support and community engagement 
  • Respite care (in-home and out-of-home breaks for carers) 
  • Transport assistance (to medical appointments, shops, social activities) 
  • Support for maintaining independence through engagement and participation 


Everyday Living

  • Domestic assistance (cleaning, laundry, essential chores) 
  • Home maintenance and repairs (gardening, minor repairs, safety tasks) 
  • Meal preparation and delivery 
  • Shopping assistance
