Community Services
BASS Care is one of the largest providers of community services within the City of Boroondara. Various BASS Care community services are available from Conochie Hall and Maranoa House in Canterbury, the Marwal Centre in North Balwyn and the Kew Community Centre.
What services are available?
BASS Care provides community services across three main areas - Social Support Services, Meals Services and specialised Dementia Day Care services.
Who can participate?
The Meals and Social Support services are primarily aimed at older people who continue to live in their own homes in and around the City of Boroondara. The programs are particularly beneficial for those people whose social network may have diminished over time due to mobility problems, illness or loss of relatives and friends.
The services also cater to younger people with a disability, carers of aged or disabled people, and anyone who is at risk of social isolation.
How can I access the service?
The first step in accessing any of BASS Care’s community services is to have an individual assessment of a person's strengths, abilities and need for support to determine appropriate service responses. This assessment is usually carried out by the Boroondara Council Health and Aged Services team and includes the development and implementation of a Care Plan. Ongoing care management involves monitoring and regular reviews by the Aged Services team to ensure an individual's needs are being met.
BASS Care clients may also be referred from other health providers and, for some services, may be assessed by the BASS Care Social Support Manager.
What does it cost?
The cost of the service depends on the activity chosen. Costs, however, are minimal as the various activities are supported by financial assistance from the Australian and Victorian Governments under the Home and Community Care (HACC) Program.
